Please refer to comments from Administrative relief request for the vicinity map and the adjacent parcel notification because those also apply to this project. If you add PCD file number to the vicinity map, it should be AL-19-022 for this project.

The same comments also apply for the application - you need to add the Parcel Number and the Existing Land Use.
