Existing bond on file is dated August 1, 2013. Bond number BLNA058388. Bond in the amount of $170,000 with no reference to height or scope of tower. Cost analysis for removal is not signed and sealed by registered professional engineer. The bond covers costs for removal of two towers on Hodgen Rd. Please provide a revised removal cost estimate that is signed and sealed by a registered professional engineer and specific to this particular tower. If the cost to remove exceeds that which the County has on file in the original bond, a new removal agreement and bond will be required. 6/15/2020 5:00:31 PM

- Please confirm Address and acreage of the lot. Change address to 11594 Hahn Rd on all documents. This illustration seems to be for the tower located on parcel # 2200000045
- Fix legal description.
- Update Letter of Intent.
- Title must be dated within 30 days of submittal date. Please update title opinion.
title opinion appears to be for the wrong parcel.
- Verify existing bond, no reference to height or scope of tower. Cost analysis for removal is not signed and sealed by registered professional engineer. - Show building setbacks from all property lines.
- Show property lines including their distances.
- No access permit on file for the property. The tower was built before 1972, therefore no charge for the permit. We still require that one is applied for to memorialize the site and can be found at https://epcdevplanreview.com/ under the "Initiate a minor access permit" link.

The wireless cell tower is located on parcel 22000-00-045. AT&T addresses for this site is 11594 Hahn Road. See permit history for this address on our web site, www.pprbd.org
Please change the address to all the pages when submitting for permit to Regional Building Department.
